Document Request forms can now be sent out of GoRev.

Document Request forms can now be sent out of GoRev from the patient portal drop down.

Within the patient account, select the patient portal icon, then request document signing.

The online registration documents will display on the left hand side, select add on the documents the patients will sign.

Once selected documents will display in the right-hand window.

Multiple documents can also be saved in a package. The load package button will send the patient all documents in the package.

Once the account has documents to send in the display window, they can be sent to the patient via sms or email.

The patient will receive an email from GoRev requesting the documents be filled out and signed.

The first step for the patient is to be verified with their name and date of birth.

A patient signature will be captured next.

Documents selected and sent to the patient will be available to sign.

Once completed, the patient will receive a completion message.

Once the patient has completed all of the documents and submitted, they will be uploaded into the file management section of the patient's account.

    • A refresh may be required if the documents do not show immediately.
