Personal Medical Leave

Severe illness or injury, hospitalization, surgery, chronic health conditions, or pregnancy/complications of pregnancy. Upstart offers 12 weeks of fully paid personal medical leave if you are unable to work. If you have exhausted your sick time or your illness/injury causes you to be out for 5 consecutive days, you should request a medical leave of absence.


How to Request a Leave of Absence (Step-by-Step)

↓ Step 1: Notify your manager

Let your manager know as soon as possible so they can plan for coverage.

↓ Step 2: Submit your leave request

File your claim through Reliance Matrix. There are 3 different ways to connect with Matrix to request a leave

Use your personal email when setting up your profile. Matrix will use this to communicate with you.

‼️ Important: You may need to apply separately for State benefits to continue to receive your full pay. Check below if your state is requires a separate leave application in addition to requesting a leave with Matrix.

↓ Step 3: Complete required actions and forms

You’ll receive a large leave packet from Matrix with:

    • Instructions and timelines
    • Required forms and certifications
    • Details on what’s needed to approve your leave and pay

Matrix will contact your medical provider to request medical certification (if applicable)

  • Your responsibilities to avoid delays or denial of your claim:
    • Follow up with your provider to ensure forms are completed and submitted
    • Complete and return your required forms to Matrix within 15 days

❗️Important: Your claim cannot be approved or paid until all required documentation is received

↓ Step 4: Stay in touch during your leave

  • Your Matrix Case Manager handles all the details of your claim and will provide status updates to you, your manager, payroll and the people team during the course of your leave
  • Respond promptly to any Matrix Case Manager requests to avoid delays in approval of time off and pay continuation
  • You can track your claim status through the Matrix app or web portal.

Step 5: Prepare to return to work

  • Reach out to your manger in advance of your return date or if it is changing from what was planned
  • Keep your Matrix Case Manager updated on your return to work date
  • You must request an extension directly through your Matrix Case Manager if your date changes
  • If your leave is for a personal medical reason, a fitness for duty form must be completed by your health care provider and submitted to peopleops@upstart.com at least 5 days before your return date.
    • If you are returning with restrictions or need accommodations, please make sure your medical provider notes it on your fitness for duty form and notify peopleops@upstart.com as soon as those restrictions are known (at least 5 business days in advance) so we can coordinate with you and your manager.

Pay During Personal Medical Leave

Your pay during leave may come from multiple sources depending on what state you live in, including:

  1. Your State Paid Family Leave benefits (weekly amounts vary based on State)
  2. Short-Term Disability (STD) insurance provided by Matrix (60% up to $3,000 per week, less State benefits)
  3. Top-Up Pay Continuation from Upstart payroll (remaining amount to reach 100% of wages)

Matrix will coordinate these programs to provide up to 100% of your regular earnings for up to to 12 weeks.

State Specific Paid Medical Leave Programs

If you reside in any of the following states ⬇️, you will be required to apply directly with your state program, in addition to applying with Reliance Matrix to receive your full pay continuation. Please be sure to discuss any questions you may have with your Matrix Case Manager. Click the state link to file your claim.

Up to $1,016.40 maximum per week

Massachusetts

Up to $1,230.39 maximum per week

New Jersey

Up to $1,119 maximum per week

Up to $1,647 maximum per week

*For California based Upstarters: The CA EDD is known to be very challenging and slow in processing leave claims. We have developed additional documentation to help with step-by-step registration, leave filing process, tips, how to transition your pregnancy claim to bonding, and contact information.

What Happens While You Are On leave

  • Your regular Upstart pay will be stopped while on leave. Pay continuation relies on your leave request being approved by Matrix
  • Upstart system access will be partially paused during leave. You will continue to have access to your email, slack, Workday, Sequoia, and Guru
  • Ramp business card and $100 access allowance are paused during leave
  • Your health benefits remain active during an approved leave
    • Your portion of premiums will be deducted during paid leave or collected upon return to work during an unpaid leave
    • If leave exceeds 6 months or you do not return, benefits will end and COBRA will be offered
  • Your equity will continue to vest while on an approved leave
