GAP - Equipment Request Form

All equipment purchases are subject to General Manager or Corporate leadership approval. Please note, your manager must submit the request via the link below.

Approved Workstations by Device Type

Germain Portal

Go to the Germain Portal
Select "Support Portal" > Submit a Request

Select "Equipment Request"

Follow the on-screen instructions to submit the request for equipment.
