How to Complete the Parent Acknowledgment Form in Clevr

If you have NOT already created a parent account in Clevr, please follow the steps at this link: Creating A Parent Account in Clevr

If you have NOT already requested access to your child(ren)'s records through clevr, please follow the steps at this link:
Requesting Access to Your Child(ren)'s Records in Clevr

  1. Login to your parent account in Clevr:

  2. Click on your child's name in the Student Record Access section.

  3. Follow the instructions posted at the top of the Parent Acknowledgement Form.
    a. PLEASE REMEMBER: To ONLY fill in the applicable "Review #" as communicated to you by the inclusive education coordinator at your child's school.
    b. Click the Inclusive Education Package button to view the PDF documents pertaining to your child.
    c. Click the check boxes next to each document type you just reviewed in the PDF (not all document types will apply to your child).
    d. Click the Guardian check box to populate your Name and the Date.
    (Note: If a second guardian is also acknowledging, they must login with their own account and click the second Guardian check box)
    e. (Optional) Add any comments you wish communicated to the Inclusive Education Coordinator at your child's school.

  4. Click the {Return to Listing] link at the top of the page to get back to the Parent Dashboard (home page).

  5. Click the Profile icon at the top of the page and select Sign out.
