GoRev LIMS Delta Checks

This guide will walk you through the delta check system built into the GoRev LIMS. What is a delta check?

A delta check is a quality control tool that involves the comparison of laboratory test results with results obtained on previous samples from the same patient. For example, in a hematology laboratory, a delta check for MCV would compare a patient's current MCV of 80 fL to their last value, obtained perhaps 12 hours ago, which was 95 fL. This significant change could indicate a real pathology change or an issue with the testing or test sample.

Configuring Delta Checks in your test compendium

Navigate to Admin --> Configuration --> Laboratory --> Edit Tests

Double click on the test you wish to configure delta checks on and look towards the bottom left of the window.

Percentage Deviation

Calculates the percentage delta from the most recent prior result for the specific patient for this specific test. As an example, if a patient had a MCV value of 85 on a test performed 6/8/2023 at 1pm central and then a MCV value of 95 on a test performed 6/8/2023 at 3pm central the percentage delta would be calculated as:

95 - 85 = 10

10 / 85 = 11.76% * 100 = 11.76

Say the two test values were 85 first test and then 70 second test. The calculation takes the absolute value to compare to delta check settings on the test.

70 - 85 = -15

-15 / 70 = -21.43%

Absolute(-21.43%) = 21.43% * 100 = 21.43

If you configure the test with a 12 in percentage deviation check it would not trigger a delta check. Configure it with anything <= 11.76 and it will.

Difference Threshold

This method will calculate the same example as

95-85 = 10

70-85 = -10

Absolute(-10) = 10

If you set the difference threshold to anything <= 10 the delta check will trigger.

NOTE: Both methods can be configured on a single test and if either method is satisfied it will trigger a delta check.

Managing Delta Checks in the normal resulting process flows

As results are received from a lab device a results status is typically set to Needs Review or a device specific status such as AU Received. When a delta check triggers this status will be overwritten to Delta Check. Same concepts apply to manual result entry.

image.png

Enter the detail results to see which specific test(s) triggered the delta check. The actual formula used to determine the delta triggering the check is displayed.

To view the details on when a specimen entered the delta check status and when your lab technician moved it forward by either re-running the test or approving as is can be viewed by right clicking on a specimen in Results Overview and clicking View History.

Note: Approving a result is not required in order to trigger a delta check. It will trigger and override any status.
