Full-Time Faculty Rank Review Applications

All nominations for full-time faculty rank reviews must be discussed with the Dean before you submit this form. You should only complete this form if you are a faculty member applying to be considered for a rank review (either stand-alone or in tandem with an RTA reappointment review) that will take place during the next academic year. This request will be reviewed by the Dean's Office. If approved, your application will be forwarded to the Provost's Office for consideration by the Provost. If approved by the Provost, the rank review will take place during the next academic year.

**All communication with faculty members about their requests and nominations will come through the Dean's Office which will make the determination in coordination with the Provost Office. All inquiries regarding the status of this application should be sent directly to the faculty affairs contact listed on the individual forms below.**

CVs are required as part of this application. Please upload your current CV to the appropriate form below. If you have any problems with the form, please email your CV as an attachment to the contact listed on the form once the form is complete, with the subject heading "CV for Rank Review Request." The application will not be considered without the CV.

Rank review leave applications are by college and academic year. The deadline to apply for a rank review is typically early to mid November for the following academic year. Please select your affiliated college below for the appropriate form:
