Add admitdate and visit location to visitstatuslog table and show in patient overview's visit section's visit status history UI

From Patient Overview's Visit section you can click the 3 bar menu. Click on Visit Status History.

This now logs changes made to Service Date (Admit Date) and Visit Location. Very useful for tracking down who changed what and when they changed it.
