Personal Medical Leave
There are several reason why you may need to take a medical leave. Severe illness or injury, hospitalization, surgery, chronic health conditions, or pregnancy/complications of pregnancy. Upstart offers 12 weeks of personal medical leave if you are unable to work. If you have exhausted your sick time or your illness/injury causes you to be out for 5 consecutive days, you should request a medical leave of absence.

How to apply for Medical Leave
- You must notify your manager prior to submitting your leave or as soon as possible if there are extenuating circumstances that would prevent you to notify them prior to starting your leave.
- To file your claim, you can file directly on the portal at matrixabsence.com, or call 877-202-0055, or via the mobile app by scanning the QR code or searching Matrix eServices.
- If this is the first time you are requesting a leave of absence, you will need to set up an account with your personal email address so that Reliance Matrix can continue to communicate with you throughout the duration of your leave.
- Within 24 hours of requesting a leave of absence, you will receive an absence packet explaining what is required to certify your leave request.
- Submit the requested documentation to your absence manager for claims processing.

Pay
Upstart leave programs include pay continuation from multiple sources. You will receive up to 100% of your pay from disability, state and/or local paid leave programs and Upstart for up to 12 weeks once certification is provided. Any additional personal medical leave beyond 12 weeks will not include Upstart top up pay. Based on your location, below are examples of how your pay will operate while on leave.
If you reside in any of the following states, you will be required to apply directly with your state program, in addition to applying with Reliance Matrix to receive your full pay continuation. You will receive instructions and additional information in your leave packet on how to apply for your state's statutory benefits. Please be sure to discuss any questions you may have with your absence case manager.

Non - CA Personal Medical
*An employee is eligible for leave under FMLA provided they (1) have been employed by Upstart for at least 12 months, and (2) has worked at least 1,250 hours during the 12-month period immediately preceding their leave. The employee's leave administrator will inform them if they are eligible for leave under these laws.
** If an employee is located in a state with statutory income replacement benefits, STD benefits will be reduced by those benefits.
CA - Personal Medical
*An employee is eligible for leave under FMLA and CFRA provided they (1) have been employed by Upstart for at least 12 months, and (2) has worked at least 1,250 hours during the 12-month period immediately preceding their leave. The employee's leave administrator will inform them if they are eligible for leave under these laws.

How to Access EAP (Employee Assistance Program)
EAP offers access to counseling sessions for stress and anxiety, medical advocacy services to assist you with navigating insurance, doctor referrals, obtaining medical equipment, transport, and help plan for transitional care and discharge.

IMPORTANT!
During your leave, your access to Upstart systems will be temporarily suspended. You will continue to have access to your email, slack, Workday, and Guru. Your access will resume once you return.
