Added a contact lookup for picking fax recipients

When using the built in GoRev Faxing Module you can now select an existing contact to automatically populate name and fax numbers. For now this pulls from Locations and Physician records only. We have plans to expand this to pull from insurance carriers, plans, and other contacts throughout GoRev soon.

Picking a record will auto populate the name and fax number as shown below

You must have Author or Collection Owner permission to create Guru Cards. Contact your team's Guru admins to use this template.
